Awareness Survey

 The school and student numbers are listed on the Master Attendance Roster.  In the box at the top of the Survey, fill in the date, school number and individual student number for each student.  It is very important that each of those fields are completed exactly the same for each child on the Preliminary Survey, the Follow-up Survey and the Long-term Survey.  The combination of school number and student number will provide a unique, confidential identifier for each student. Do not put students' names anywhere on the surveys. 

 

 Please make sure to remind your students to turn to the back page and complete both sides of the survey.  Copies of the Long-Term Survey will be sent to you the last week of April and should be completed before May 7 or you can copy the survey from here.
